Do you get informed if money will be taken out of your tax refund?

If you know that you have some unpaid medical bills ( i live in texas ) , some unpaid credit cards ... can they take from your tax refund and if they do, do the hospitals / creditors / IRS normally send you a letter letting you know BEFORE you file?
